An Invisalign member must remain a plan member for the duration of the treatment to retain discount plan benefits.

The patient agrees that the Office Fees stated must be paid in full at the time services are rendered.
Any service not paid for at the time of service will be billed out to the patient at our usual & customary fees (UCR)
Program Guidelines
• The patient's portion of the bill is due on the day of service.• Percentages/Fees are based on the Usual Customary & Reasonable Fees (UCR)
• There will be a $50 reinstatement fee if your plan lapses.
• Cannot be used in conjunction with another dental plan or financing program such as Care Credit.
• No refunds of premiums will be issued at any time if the participant decides not to utilize the dental plan.
• NON-REFUNDABLE
Plans Exclusions & Limitations
This program is a membership plan, not a dental insurance plan.
It cannot be used as follows:
• In conjunction with another dental plan, dental insurance,
or financing programs such as Care Credit • For treatment which, in the sole opinion of our doctors, lies outside the realm of their capability.
• For referrals to specialists.
• Hospitalization or hospital charges of any kind.
• For costs of dental care which are covered under automobile medical.
• For services of injuries covered under workers' compensation.
